COVID-19 acknowledgement document

I have been fully informed that as a requirement of travel by the regulatory authorities of my country of destination including transit I must, where requested, present/submit valid COVID-19 documents (for example: PCR test results, health declarations, vaccination documentation, passenger location form or other COVID-19 related documentation).

I acknowledge that sharing my valid COVID-19 documents with Qatar Airways is necessary to comply with legal obligations to which Qatar Airways is subject (e.g. ensure all passengers traveling to a specific destination have with them the necessary COVID-19 documentation required to enter to that destination).

Qatar Airways will process my personal data under a legal obligation and/or under a legitimate interest and reasons of public interest in the area of public health such as protecting against serious cross-border threats to health.

Qatar Airways will keep my personal data confidential and will share it only with the competent authorities where required by law. Qatar Airways will retain a copy of my COVID-19 documents for a limited period with the sole purpose to demonstrate compliance with applicable legal obligations.

Where applicable, if the release of this information to Qatar Airways is on behalf of a child I acknowledge and agree that I have the legal capacity to do so as a parent or legal guardian of that child.  I confirm that I have read and fully understand this document before signing below.
